Job Description Hospital Pharmacy & Medication Safety
Key Responsibilities:
Manage medication safety, formulary management and rational medication use.
Monitor antimicrobial usage, medication errors and ADRs; support pharmacovigilance activities.
Conduct prescription, pharmacy and compliance audits.
Verify chemotherapy protocols and participate in chemotherapy mixing audits.
Ensure safe handling and storage of narcotics, psychotropic and high-risk medications.
Support QMS, SOP, CAPA and regulatory compliance initiatives.
Provide evidence-based drug information to healthcare professionals.
Participate in PTC/hospital committee meetings and quality improvement initiatives.
Monitor product quality issues and prepare quality indicators/reports.
